Resident Liaison Program at Evergreen House Health Center gives patients extra attention
Posted on Friday, October 30, 2009Evergreen House Health Center in East Providence, R.I., recently implemented a resident liaison program, pairing long-term care residents with facility associates who take extra time to make sure their residents are being cared for.
Each resident room is assigned its own liaison, an associate who has worked with the facility for at least one year and has gone through training in the program. The liaison communicates with his or her residents daily, checking in to see how they are doing and if he or she can help with anything.
“We’re able to personalize our one-on-one contact,” said Lisa Santos, marketing director and resident liaison. “It’s made for better relationships.”
The liaison program, only a few months old, has already identified several resident needs, which have been or are being addressed. The facility is getting new pillowcases, for example, and one liaison discovered a resident with no family in the area needed new underwear, so she went out and bought some for him. Another liaison discovered that a resident needed a belt, so he went and got one for the resident as well.
“Some of these may seem like little things, but they are important to the residents, so they are important to us,” said Santos.
Residents are not the only ones who benefit from the program. The liaison communicates with the residents’ families at least once a month regarding their loved ones’ care and wellbeing.
Families can also contact their loved ones’ liaisons if they have any questions or concerns. Every other week, liaisons meet with the facility leadership to discuss how the facility can best meet the residents’ needs.
“The program has gotten a lot of good feedback from the families,” said Santos. “They feel like they have more support.”
