Life Care Center of Old Hickory Village, in Old Hickory, Tennessee, recently achieved a deficiency-free health survey.

To pass the health survey each year, nursing facilities must meet hundreds of criteria. These survey visits are unannounced and address not only nursing care and resident safety, but also patients’ rights, food services and administration. Surveyors spend several days in the building reviewing paperwork, observing resident care and interviewing residents and associates.

“Obtaining a deficiency-free survey is something not many do in their entire careers, but the team and I at Life Care Center of Old Hickory Village have been a part of two in the last three years!” said Dale Slivka, executive director. “The team really worked hard for this, with the support of the division and regional teams, and the ultimate compliment was paid.”

State surveyors visited Life Care Center of Old Hickory Village from May 13-15, 2019.
