Life Care Center of Evergreen earns Medicare’s five-star rating

Life Care Center of Evergreen, Colorado, recently earned a five-star rating from the Centers for Medicare and Medicaid Services.

The rating system for CMS is designed to help people choose the best care for loved ones. The ratings are based on associates, health inspections and quality measures such as weight loss, falls and pain management.

“We are very proud to be rated a five-star facility,” said Amber Carlson, executive director. “This past year has been full of many challenges, but the associates at Life Care Center of Evergreen have remained committed to providing the highest quality of care for our residents. Our team takes great pride in serving our mountain community as family.”

The five-star rating includes data through December 2020.
