My mother Barbara is receiving excellent care at this facility. She has enjoyed the staff, the quiet and clean room, the friendly roommate and good food. They worked closely with Medicare to ensure all her service was covered and provided some nice thoughtful gifts when she arrived.

1⭐️ cause I have to. I’ll preface this with, this review is based on my Father and I’s experience with the facility & is strictly my opinion of the care, staff & intent received during while detailing the direct reasons I formed the OPINION/REVIEW I have. I would not send a fruit fly on hospice to this facility. The majority of the staff were lacking comprehension & appropriate implementation of physician orders, they missed medications (discharged 1/16 with antibiotics & probitocs unfinished from December [7 Day Script])), inability to provide adequate, intentional and resident specific care in relation to cardiology and pulmonology, heard and witnessed staff speaking negatively to/about the residents, allowed for call lights to go unanswered (at times for about an hour), call lights also not consistently left within reach for resident utilization, consistently ran out of medications (discharged without pain & psychological meds due to them ‘running out’, linens were consistently dirty/unchanged after soiling with just a bed pad being placed over the dirty/soiled spots, DON “leads” poorly and does not assist her staff when they are short staffed (which is daily), she, the DON Tammy also admits that the facility “failed my father” - her exact words and that they could not care for him like he needed; I attended 3 care plan meetings for hopeful resolution & forward movement without permanent change— unfortunately in healthcare you do not get to pick your patients or residents temperaments or conditions and instead have to adapt and adjust care provided to meet the individualized needs of each patient/resident {I am in healthcare myself and would never treat a patient they way my Father was treated and allowed to be treated by the DON and executive director - Jared, of who was made aware of all of my concerns on 2 separate occasions at least), menus repeat with lack of food group incorporation, etc. etc. — my final deciding factor to remove my father from the facility was when he allegedly fell out of the bed on 12/11; which, I say allegedly because the ER Doctor charted that he is unsure how the patient was injured because what was reported to him by EMS from the facility did not align with the injuries sustained & when I requested the documents and incident reports from the facility from the ‘fall’ both verbally and in writing the facility would not provide them & still has not to date - nor did they ever respond to his PCP with said documentation or an incident report of what occurred. During my fathers 6 month stay I reported the facility to APS and DHHS twice advocating for my Father and the other residents and the care they deserved/deserve. Now, there is some positives I’d like to note - the therapy team is excellent, Ashley, Gina and Katie - top tier, very intentional practices and a demonstrated high regard for the residents, there is also some high quality nursing staff amongst the mess such as Larry, Melissa, Pavi, Florence, Rhodwell, Quita, Sandy, Belinda & Aubree. Additionally, their housekeeper Lee and the new one they’ve recently hired did a phenomenal job at keeping his room clean!
