Gerald at Life Care Center of Elyria

Gerald was admitted to Life Care Center of Elyria for intensive short-stay therapies after a prolonged hospital stay. He required comprehensive rehabilitation to regain basic functioning and to prepare for a safe return home.

When he arrived, Gerald was dependent for all self-care, bed mobility, and general functional mobility. He was unable to stand or walk and needed a Hoyer lift to make transfers. Because of a swallowing impairment, he required a tube for all food and drink.

The therapists at Life Care Center of Elyria created an intensive interdisciplinary program for Gerald that combined physical, occupational, and speech therapies to improve his strength, balance, and endurance and address his swallowing difficulty.

Despite a few medical setbacks, Gerald’s treatment progressed smoothly, and he remained determined to regain his independence. Along with his determination, strong support from his family motivated him to keep working hard and meet his goals.

Gerald discharged with restored bed mobility, a regular diet, and the ability to make transfers and walk over 50 feet.

After returning home, Gerald continued his journey with outpatient therapy at Life Care Center of Elyria, further enhancing his strength, ability, and confidence. Now he can walk 150 feet!

Ladeane at The Woodlands

Ladeane admitted to The Woodlands for complications related to COPD.

At the time of her admission, Ladeane needed supervision with activities of daily living, transfers, and walking. She also had severe vocal tremors that distorted her speech.

Her rehab program included physical and occupational therapies three times a week for six weeks and speech therapy three to five times a week for eight weeks.

In physical and occupational therapies, she focused on rebuilding her strength, restoring her balance, and expanding her safety awareness. In speech therapy, she and her therapists targeted her vocal tremors, using the SPEAKOUT vocal exercise program.

After meeting all her goals, Ladeane was able to discharge to Silver Birch Assisted Living Facility, fully independent with all activities of daily living, transfers, and walking while using a rollator. Her vocal tremors were mild, and her speech was much clearer.

Leonard at Life Care Center of Saint Louis

Leonard needed short-term therapy services after complications from a femur fracture.

Upon his admission to Life Care Center of Saint Louis, the physical, occupational, and speech therapists evaluated Gerald and determined he needed assistance with bed mobility, transfers, bathing, dressing, and walking.

There were also impairments with his swallowing, problem solving, and safety awareness.

Leonard’s treatment involved physical, occupational, and speech therapies five days a week. Physical therapy focused on his gait and balance

training, while occupational therapy focused on retraining in activities of daily living and transfers. Speech therapy targeted safe swallowing strategies and cognitive training.

Upon his graduation from short-term therapy, Leonard could perform daily activities and transfers with supervision. He also walked over 150 feet with a walker. His swallowing ability was normal, and his cognitive function had improved.
