Statement from Heritage Park Care Center regarding COVID-19

Skilled Nursing & Rehabilitation in Carbondale, CO

Updated: Friday, January 8, 4:30 p.m. MST

At Heritage Park Care Center, the safety of our residents and peace of mind for family members are our highest priorities.

We have had two residents test positive for COVID-19 on Jan. 4. Both are being cared for in a separate area of our facility. One associate has also tested positive and will not return to work until CDC guidelines are met. As of now, associates are tested twice per week, and residents are tested twice per week.

We continue to follow the guidance provided by the Colorado Department of Health, the Agency for Health Care Administration and the local health department.

Our primary concern is for the health and safety of our residents, nursing staff and other care providers. Our residents are monitored multiple times day and night for any symptoms or concerns. And every associate is screened upon arrival and at the end of their shift, including checking temperature and monitoring for any symptoms or concerns.

We recognize that family and friend relationships are crucial to our residents’ well-being, and although we cannot have in-person visits at this time, we are always happy to coordinate phone calls, video calls and window visits. To set up a call or window visit, please call us at (970) 963-1500.

I am thankful for the amazing team we have at our facility and their dedication to caring for our residents.

We will continue to post updates and communicate changes in our COVID-19 numbers with our residents, their families and our staff.
