Alma at Rivergate Terrace

First, we would like to wish Alma a Happy 103rd Birthday! Alma came to us a few weeks shy of her birthday after being hospitalized for congestive heart failure.

She had been living in an independent living facility and was independent with all her self-care tasks and simple meal preparation. She preferred to use her wheelchair as her main mode of transportation.

Upon admission to Rivergate Terrace, Alma required moderate assistance for bathing and lower body dressing and needed help with meal preparation and light housekeeping.

She required moderate assistance for transferring and walking 10 feet with a walker.

While at Rivergate Terrace, Alma worked diligently with physical, occupational, and speech therapy. In occupational therapy, she quickly reclaimed her ability to dress and use the restroom on her own.

One of her biggest personal goals was to regain the ability to make her toast and coffee in the morning––which she did!

And thanks to physical therapy, Alma is propelling her wheelchair on her own and making transfers confidently. She is now exceeding her previous distances in her wheelchair and even walking 150 feet with a walker!

Alma was able to discharge home to her independent living community the day before she turned 103––just in time for her birthday party!

Rivergate Terrace

Carl at Life Care Center of Elyria

Carl came to Life Care Center of Elyria after being treated for medical complications that resulted in pain, weakness, and vomiting.

Upon admission, therapists developed for him a comprehensive rehabilitation program that included physical and occupational therapy.

Initially, Carl had weakness and limited ability to complete daily functional activities. He also had serious balance issues and was at high risk for falls.

The physical and occupational therapy teams formulated a dynamic, integrated treatment to improve his ability to complete his daily routines safely and independently.

Carl made great progress soon after beginning his personalized therapy program, regaining his ability to walk, complete self-care tasks, and make transfers independently.

He also improved his balance, reducing his fall-risk status, and increased his overall strength.

Life Care Center of Elyria

Gregory at Life Care Center of Morgan County

Gregory admitted to Life Care Center of Morgan County after being treated for a hip fracture and a stroke.

At the time of admission, Gregory needed moderate assistance with activities of daily living and functional mobility.

Gregory received therapy five times a week, focusing primarily on strengthening his lower body and improving his balance for safe walking. Occupational therapy focused on strengthening his upper body and increasing his safety awareness to restore his independence with daily activities.

Speech therapy targeted his speech and communication deficits caused by the stroke.

Gregory was able to discharge home with his independence restored, though he still required outpatient physical, occupational, and speech therapies to further ensure his full recovery.

Congratulations to Gregory on all that he accomplished during his short-term inpatient and outpatient therapy!
