Patricia and Tom at Life Care Center of Elyria
Patricia and her husband, Tom, both experienced falls that resulted in fractures.
After their surgeries, they transitioned to Life Care Center of Elyria for comprehensive rehabilitation to improve their strength and maximize their independence.
At the time of admission, they both required assistance for all their functional mobility and self-care tasks. They also required education to adapt and manage their new daily routines for their recovery.
Physical and occupational therapists evaluated their needs and formed a dynamic, integrated treatment to help Patricia and Tom improve their functional ability and discharge back home safely.
They participated in their individualized plans of care and quickly made great progress.

Anthony at Parkview Nursing and Rehabilitation Center
Anthony came to Parkview Nursing and Rehabilitation Center after a left hip replacement.
He needed to regain his strength so that he could resume his activities of daily living without assistance.
He actively participated in skilled physical and occupational therapy to increase his functional independence and return home safely.
His personalized rehab plan addressed and improved his balance, gait, and ability to navigate stairs safely. Additionally, Anthony worked to improve his ability to bathe, dress, and groom independently.
Anthony achieved his goals and was able to go home with his independence restored. He no longer needs a walker, though he may require a cane when walking outdoors.

Brenda at Life Care Center of Morehead
Brenda was in a car accident and sustained multiple fractures. After surgery, she came to Life Care Center of Morehead for short-term therapy.
When she arrived, Brenda could not stand and required assistance with all daily activities and transfers.
Five days a week she received physical and occupational therapy, improving her knee flexion and extension, strengthening her upper and lower extremities, and practicing transfers and completion of daily living activities.
Once she was able to bear weight, Brenda worked on standing tasks and gait training.
After less than three months, Brenda was able to dress and transfer on her own and walk 125 feet using a rolling walker. Her overall strength had improved greatly.
