Ms. Moore was admitted from St. Louis Hospital due to COPD exacerbation, pulmonary embolism and left lower extremity deep vein thrombosis.
Ms. Moore needed moderate assistance with dressing, bathing and using the bathroom. She required contact guard assist for bed mobility, sit-to-stand, transfers and walking 10 feet with moderate assistance. She was found to have moderate-to-severe cognitive impairment and had complaints of coughing when eating.
She was treated by physical, occupational and speech therapies five times a week, focusing on strengthening, transfer training, activities of daily living retraining, ambulation, cognitive strategies and safe swallow strategies.
"Therapy helped me walk again and care for myself. I would come back if I ever need therapy again. All of the therapists have been so nice and helpful. If I could give you 10 stars I would!," Ms. Moore said.
At the time of her discharge, Ms. Moore was able to perform activities with modified independence, with the exception of bathing with supervision, and she was able to walk 150 feet using a walker with supervision and perform bed mobility and transfers independently. She was also able to safely eat an easy-to-chew diet.
